What confidential  information we need from you to complete an Occupational Therapy Assessment: 

Name/ address / contact details / gender / next of kin details 

Information regarding your medical conditions and if relevant to the service we are providing we may ask about any mental health conditions. 

What do we use your personal data for? 

It is important for us to gather accurate information in order to provide a thorough and appropriate assessment. The information gathered will help guide our clinical reasoning and provide you with a quality and safe service. 

We can not be held accountable for the outcome of our assessment if the information you provide us is not correct.
